How to Join a Health Net Medical Network?


We’ll need some information from you before we start the application process to confirm that you meet the minimal eligibility standards for credentialing. Select the credentialing option, and someone will assist you. If you meet the basic criteria for credentialing, you will often be notified during this conversation. If you are a facility or an auxiliary provider, we will need more information from you than what is required on the Provider Information Form. Select the credentialing option, and the individual will notify you of the following steps in the application process.

We will submit your credentialing application

If you fulfil the basic requirements for credentialing, you will receive an email with a registration packet and all of the information you need to get started. If the application information is already available on the Council on Affordable Quality Healthcare® (CAQH) or the One Health port/Med versant® websites, we will access it electronically with your permission to obtain the majority of the data we require. If your state requires it, we will accept a state application that you fill out, sign, and return to us along with the relevant paperwork.

  • Within 100 days, ensure that all appropriate fields have been completed and signed

We need that you submit a full application form with the necessary information, regardless of how you apply

Requirements to be furnished

  1. A state medical license or an authorized professional license is required.
  2. Certificate from the Drug Enforcement Administration (DEA)
  3. Certificate for Controlled Dangerous Substances (CDS)
  4. Clinical privileges at Health Net-affiliated hospitals
  5. Status of board certification
  6. Education and training Professional experience
  7. Activities prior to sanctioning
  8. History of malpractice claims
  9. Sufficient malpractice insurance

  3. Each practicing state needs one
  4. Must be a member of the medical staff in good standing and meet the necessary state and medical specialty standards
  5. The amount of professional liability coverage will vary depending on market standards and medical specialty
  6. Include relevant training in the specialty for which you are applying, as required by applicable regulatory organizations

Other Information

  • IMPORTANT: If you do not have a DEA or CDS certificate, please complete the DEA certificate.
  • Include an explanation for any gaps in employment that are longer than six months.
  • On the application, penalties information must be disclosed.

Keeping you updated during the registration process

As soon as we receive the application material, we will begin the credentialing process. This typically takes 45 to 60 days to finish. We will send emails to the following addresses at this time:

  1. Confirm that your application has been received.
  2. Request any missing documents or application information. If we do not get the missing information, we will notify you through email that your application has been closed.
  3. Keep you informed of the status of your application in our credentialing verification process, including any delays that may cause the process to take longer than the average 45- to 60-day turnaround time.
  4. Provided the following that you have been accepted and credentialed as a network-participating provider with your effective date, or inform you that you have not been approved.

If you are authorized, your provider information will be added to our directories and claim systems within 10 business days.


When you need to be re-credentialed, Health Net will send you an email asking you to confirm that your CAQH information has been updated and to re-sign it. You will receive up to three email reminders. If you do not log into CAQH to validate and sign your information, you will be dropped from the Health Net network.

You will be directed to apply online using all of your established credentials. Every entity you enter will be verified and cross-checked.
